Schedule an appointment:

You can schedule a consultation by contacting us at 940-566-5714. If at any time you are unable to keep your appointment, please notify us at least 24 hours in advance so we can reschedule your appointment at a more convenient time.

Patient Forms:

Please fill out the following forms online prior to your arrival. Once completed, you may call our office to schedule your appointment. These forms will be sent automatically to our office and can be downloaded for your records. This will save time at registration and get you in the therapist’s office more quickly. Please fill out as completely as possible.

• Notice of Privacy Practices

Billing:

We accept cash, checks, Visa and MasterCard and will file all of your claims for you.
We can also set up a payment plan for any remaining balance.
WE ALWAYS, ALWAYS, refund any money to you once your account is settled with your insurance.

Insurance:

We will be glad to discuss your deductible, co-pay and insurance plans with you and will verify your benefits before making your appointment.
