Owner  

Dan Proctor, P.T.  Owner and Administrator since 1991.  Licensed Physical Therapist.  Preliminary course work from Baylor University and Physical Therapy Degree from University of Oklahoma Health Science Center completed in 1984.   Management and Administrative experience since 1986.  Areas of Expertise:  Knee, Shoulder and Ankle Injuries; Sports Injuries; General Orthopedics.

 Medical Licensed Staff

                    

Krista Isom, P.T.  Licensed Physical Therapist.  Bachelor of Science in 1998 from TWU and MPH in 2002 from Texas Tech.   Areas of Expertise: General Orthopedics, Neuro, Acute Care, and Workman's Comp.

 

 

Beth M. Cox, O.T.A.  Licensed Occupational Therapist.  Bachelor of Science in Occupational Therapy from Texas Women's University completed in 1994.  Area of Specialization: Spinal Cord Injury, General CNS Dysfunction and Rehabilitation, Hand Therapy, and Work-related Injuries.

 

                      

Sarah Velazquez, O.T.R.   Licensed Occupational Therapist.  BS & MS (2002) from Texas Tech.   Areas of Expertise: Hand, Wrist & Shoulder injuries; General Geriatrics & Pediatric Rehabilitation.

 

Tom LaRussa, Ph.D., LPC,  Licensed Professional Counselor and Independent Contractor with DS&PTC since 2000.  Bachelor of Science in 1978 and Masters of Science in 1980 both from Northern Illinois University, and Ph.D. in Counselor Education in 1995 from University of North Texas.  Functions as integral member of the Working Hardening Treatment Team and consultant regarding Medicare patients.

Therapy Aide Staff

Most of our aide staff are currently pursuing or have completed science degrees from a four-year university.  Most are planning to apply to Physical Therapy or Occupational Therapy School as they finish their undergraduate studies.

Business Office Staff

Jo Ann Nuzzo – Insurance Manager since 1995.  Handles Patient Accounts, Billing, Accounts Receivable, and Collections.

Kerry Kruger – Data Coordinator since 2006.  Handles New Patient Information, Pre-Authorizations, Referral and Verifications. 

Sylvia Koleber – Head Secretary since 1995.  Schedules appointments, coordinates charges and payments.

Helen Stewart – Business Manager since October 2002.  Handles Bookkeeping, Employee Benefits, Payroll, and Daily Operations.
